With these adjustments, I can get the document to show up in the appropriate object store and folder in FileNet, but it does not create a Case.

While you can start a new case instance with a service call out of Datacap, I agree with Mike Prentice. There are facilities in FileNet and Case Manager to do this for you. The section that Mike is referring to int he case definition is "Starting document class", which in the first screen of the Case Type definition in Case Builder:
