I have an ongoing issue where when I click on the IT Management -> Projects -> Project Details the report is filtered on a Project Name that I had applied a filter some time ago. I can not get rid of the name. I have tried promoting DEV to STG and STG to PROD with the Project name unfiltered but nothing works.

Looks like this is some issue with the OOTB reports. So, Apptio has a way by which you can revert the OOTB report to it's original state. Here's the reference:

You may also want to check the report versioning if you changed the access rights to the report
