I just finished a workshop with an insurance client. A key topic was Post-Incident reviews and the importance of a blameless culture. If people are not held accountable for any wrongdoing or mistake, they will share mistakes to help prevent others from making the same mistake. Recognition and commitment to a blameless (or better: blame-aware) culture is crucial for effective post-incident reviews, resulting in improved operational efficiency - but also in better employee morale and trust.
