I've turned auto approval on and selected the specific auto approvals I want - Read, Write etc. However, no matter what I try, I'm always prompted for approval. Is there more to turning on Auto Approval?

Did you check the advanced options for the auto-approve actions (under Settings > Chat), to make sure you are not in one of the cases needing additional approval (i.e. reading files outside the workspace) ?
