The major/minor device number will tell you what lvm object is being referred to.  In the errpt, the values are in hex.  In /dev, the values are in decimal.  I'm fairly sure that the device you want to look at is major 000A = 11, and minor 000C = 13.  These values should be in the 5th and 6th column of a long listing of /dev.

I'm assuming that this is a production system, so I would highly encourage you to call IBM Support...
