We've had this issue with multiple users, and it happens exclusively with Chrome. Completely uninstalling and re-installing temporarily fixed the problem, but it came back after a few weeks. When Chrome is the default browser, I've been having users create a desktop shortcut that opens Cognos in Edge:
Right click on your desktop and select New > Shortcut.
Change the server and domain name in the string below to the appropriate value for your installlation, then copy the entire string (including the quotation marks) and paste it into the shortcut as the location of the item.

Click Next and give your shortcut a unique name.

This issue was resolved. We ended up having to complete uninstall the browser, delete the entire Chrome folder, and then reinstall the browser. After that, the user did not experience any more issues using Cognos in Google Chrome.
