You need to run kernel debugger with subcommand options stat,f and stack.

1. kdb

This returns a (0)> prompt
Type stat on prompt and enter
Do same for 'f' and 'stack' subcommands

Post the outputs.
Also if you need to see options description type help on (0)> prompt.

All I can say is to run diagnostic tests for your drive.

Follow below path:-

diag >> Diagnostic Routines >> Problem Determination >> (Select) cd0 >> F7

Let me know if it throws some errors.

If I installed 6100-00-01, then using upgarde DVD to update to 6100-02-01, looks good, but after OS reboot, halting again with same information.

1) lscfg -vl cd*

Examine the data that is returned. If Part Number is 04N2964 and ROS Level and ID is less than or equal to 1_04 (for example, 1_02, 1_01, or 1_00)
Then you need to upgrade CDROM firmware again in case even if you have upgraded.

2) Also minimum firmware required for AIX 6100-02-01 is 3F041029.

Also find out more such solutions in AIX 6100-02-01 hardware compatibility pdf.

System firmware, now is 3F080425,
DVD is Slim IDE DVD-ROM Drive / DROM00205, now microcode is NR38,

both are latest version which I could find.
