I tried multiple browsers and private mode in each with the same result. I had a colleague do the same and they got the same result.

Did you try with another browser, in private mode? Or with another user?
You might also want to run Cognos Analytics with customized extensions and views disabled. To do this (just for a test), start Cognos Analytics by typing the URL:
<webserver_name>:<port_number>/bi/?factoryMode=true.
Cognos Analytics will start with all extensions disabled. You can create a new dashboard and try again to add a new tab to see if it solves your issue.
