Hi I am currently doing my dissertation analyzing my likert scale data to find  the relationship between service quality and customer satisfaction .  I would like to present it under the second picture. Can anyone advise me how to use regression analysis to calculate this ? I am not sure where to add the control variables eg gender,age education etc and would I add independent variables under block 1 of 1 .

Issue I am having is previous researcher had tangibility , reliability etc as its heading for independent variables  so not sure if i would need to group my questions under each heading as i have 4-5 survey questions per heading

You can put the variables in blocks, but they are cumulative, which is probably not what you want.  If they are factors, you can use GLM/UNIANOVA or LINEAR to get results for whole factors.  If they are scale variables, they would have separate coefficients unless you use something like principal components and take the first factor for each group as a regressor.
