Regarding implication, Sterling allows you to override document storage at BP level. So as long as all BP are configured with default storage you don't need to update any BP else you would need to update respected BP to change document storage to system default.
for performance, it depends on file size, if Sterling integrator is dealing with large file, then it would be better to go for file system storage for smaller file DB storge works best.
regarding DR strategy, yes you would need to plan replicate documents to DR server.
Its recommended to test it well before applying change in produciton.
