Yes, ITX with the Pack for Healthcare EDI is a great way to mask PII and PHI and transform HIPAA EDI data. You could do this as either one or two transformation steps.
If your goal is to start with an EDI file and end up with an EDI file where the only change is the masking of some data then a pass-thru type map will be easiest. Earlier in this thread Paul Brett posted a simple example of one such map. Variants of such a map can include rules that are as complex as you need them to be.
If your goal is to retain intermediary CSV files than it is also possible to map from the EDI to the CSV then to the new EDI. Your approach will be shaped by your requirements. But yes, ITX is the right tool for such data masking and transformation, and the HIPAA pack provides artifacts to get you started.
