I've installed this on several instances as well (SQL Server 2016 and CA v11.0.7) and haven't had any issues with DB connectivity. A couple things that might help though:
1. I added the Audit_EXT tables to existing Cognos Audit DBs
2. I don't remember all the options for DB types but something tells me your issue is likely related to the MS SQL Server (integrated security) choice - if there is just a regular MS SQL Server option, give that a try.
The audit extension has its own security, completely separate from whatever your Cognos environemnt uses...We use AD with SSO enabled for Cognos but there is nothing equivalent to that for the Audit EXT that I'm aware of.
One final suggestion would be to open your main Cognos Configuration interface and verify how you have the connection to the main Audit DB configured. If your primary audit DB is working as expected that configuration could give you a clue as to how this connection should be set up. Obviously, that's assuming you're not trying to set up a brand new DB just for the Audit EXT tables.
