This can occur when you assign an existing id to a new instance, and before flushing attempt to load the existing instance for that id.

Have you performed backup restoration recently. From the look of the error you have presented, it seems to a known issue with deploy changes after restoring a config backup.

I suggest raising a Support case to investigate and fix.
