Here is a list of generic failure codes we used at a water/wastewater facility. We added a component field in the failure reporting process to capture that as well. Techs were happy to report failures and not have to scroll through hundreds of options. Data analysts were able to quickly identify exceptions down to specific components. It ended up being a good fit for them.
If I remember correctly most, if not each, of the causes and remedies where included in every problem in the hierarchy because they were so generic.
