    Hi Guys,
    We need Any Generic Failure Codes / Hierarchy data available especially which can be used in Municipality (e.g. facilities, water/wastewater).
    If not specific for municipality, then any generic data, which can be used as a starting point
    MS Excel or MS Word data will be highly appreciated

    Here is a list of generic failure codes we used at a water/wastewater facility. We added a component field in the failure reporting process to capture that as well. Techs were happy to report failures and not have to scroll through hundreds of options. Data analysts were able to quickly identify exceptions down to specific components. It ended up being a good fit for them.

    If I remember correctly most, if not each, of the causes and remedies where included in every problem in the hierarchy because they were so generic.

    Problem AIR Abnormal Instrument Reading
    Problem AOP Abnormal, Erratic, or No Output
    Problem BRD Breakdown
    Problem DOP Delayed Operation
    Problem LEK Leakage; Internal, External, or Process
    Problem FTF Failed to Function (Open, Close, Regulate, Start, Stop) on Demand
    Problem NOI Noise
    Problem OHT Overheating
    Problem OWD Operates Without Demand
    Problem PLU Plugged/Choked
    Problem VIB Vibration

    Cause OAML Part Out of Adjustment, Misaligned, Loose
    Cause DCSU Dirty, Clogged, Stopped Up, Frozen
    Cause LIQU Incorrect Lubricant, No Lubricant, Insufficient Lubricant, Contaminated, Temperature
    Cause DEWA Defective, Wrong Application
    Cause POWR No Power, Overload, Insufficient Power
    Cause HEAA Human Error, Abuse, Accident
    Cause WEAR Wear, Worn, Corroded, Cracked, Eroded, Aged
    Cause WGSZ Wrong Size
    Cause SWER Program Error
    Cause EXTE External Force, Environment

    Remedy AJUST Bringing any out-of-tolerance condition into tolerance
    Remedy CLEAN Clean, dry out, etc. to return to operating condition
    Remedy LUBRI Lubricate, correct fluid levels
    Remedy CHECK Investigated, but no maintenance action performed, or action deferred. Able to restart/reset.
    Remedy MODIF Replace, renew, or change item, or a part of it, with an item/part of different type, make, material
    Remedy OVRHL Major overhaul, rebuild
    Remedy REFIT Minor repair/servicing activity to bring back to an acceptable appearance
    Remedy RPAIR Manual maintenance action performed to restore an item to its original appearance or state
    Remedy RPLAC Replacement of the item by a new, or refurbished, of the same type and make
    Remedy COMBI Several of the above activities are included
    Remedy OTHER Maintenance activity other than specified above
    Remedy RCHRG Recharge
    Remedy TRAIN Provide training to user
