Thanks all, I ended up re-downloading the iso and mounting it on our admin LPAR and copied over - updated 2 VIO servers and alls good

While updating to 4.1.1.0, there will be certain Requisite Failures which can be ignored. Detailed list is mentioned in the release notes at https://www.ibm.com/support/pages/node/7178052?mhsrc=ibmsearch_a&mhq=vios%204%26period%3B1%26period%3B1
You may try downloading the update images one more time.

We attempted to upgrade our VIOS 4.1.0.21 servers to 4.1.1.0.0 but it failed miserably. The servers got stuck at code : 0554
There were a lot of MISSING REQUISITES as well and when it went to reboot, there was an issue with /etc/shutdown :

Our command run as padmin : ioscli updateios -f -accept -install -dev /mnt/$UPDATE
