In the Extension Management interface look for the failed LSM upload, delete it. Ensure any other instances of LSM in the Extension Management UI is deleted as well. Then, clear browser cache, close browser, re-open browser, and try uploading/installing the LSM app again.
The troubleshooting steps I am suggesting are similar to the steps in this article (different symptom, but same steps to try and resolve issue):
