    There are lots of permitations and combinations with disks and AIX Support can recommend the the best way to save any data that might still be available, clean up your volume groups and filesystems plus checking the ODM and then get your new disk online.

    Also on older machines, some disks may not be hot plugable.

    For investigating your disks use: lspv, lsvg and lsconf is useful.
    You may be able to migrate data off the disk with migratepv.

    If it is really dead then you need to tell AIX to forget it. This is tricky depending on its state and if it is in a volume group with other disks or not and if it was holding logical volumes that are also partly on working disks.

    Like I said use AIX Support - they do this regularly and know what to try first.

    Getting the disk online is simple: reboot or use the root command cfgmgr and then put the disk in a new or current volume group and you can start using it.
