    Also how the disk is configured. Is this part of a RAID array, or just a single disk?

    Useful info would be an errpt, diagnostic output - hopefully with an SRN - and some LVM data: "lsvg -p VG" etc.

    Replacing the disk is probably trivial, but recovering any data on that disk may be more of a problem.

    You could try reading up on it in the LVM A-Z Redbook, (there are two volumes to this and I forget which one you'd need).
