This might be related to TN global issue,Please install these released fixes located in www.webmethods.com/tnresolution.

We were using 4.6 version and we also had same problem and we made patch fix for this and it’s working now. Couldn’t remember the exact patch name but you can surely find some interesting threads in this forum.

This is a global problem which encountered in 2004,see this link for downloading fixes and install it on IS/TN server depending on your OS.
