Further research lead me to create my own PDF form using Libre Office - 'Example.pdf'.

The attached example can be used to as a simple demonstration of the adapter with ITX 9.0.0.2 and above.

I used the PDF importer to create the 'Example.xsd' and 'Example.names' files.  I used the Eclipse based Design Studio (in XML Perspective)  to create the 'Example.xml' input file.

The map is a simple one-to-one mapping and creates the fully populated 'Output.pdf'.
