    looks like you have more than one problem.

    first fix the I/O error. that's an indication of a severe problem like hw failure (disk/cable/adapter) or maybe filesystem damage.

    then deal with the mkszfile error (mkszfile is being called by mksysb and in your case encounters an unexpected condition). often the reason for this is a "non-common" setup, like a logical volume labled "jfs" that contains a jfs2 filesystem...

    j.gann is correct. You might have a bad disk. Then you can get rid of those two filesets showing by lppchk -v if not needed. But that shouldn't stop you to take mksysb. Run errpt -a and look for the errors
