I figured it out
I had to allow Chrome in polices > Browser, then block all sites with (*) and allow the Google Form with its URL

Hey,
so what you are looking for here is Single App kiosk mode. On your Android policy under the Kiosk section select edit and then set the mode to Single app

Enter the app id for you Google form and then save and publish the policy.
