Based on my experience, when the import process works correctly, the following sequence of messages is typically displayed. Judging from the error message, it appears that the download from the cloud storage has not started yet.
1.Importing image file from cloud storage ...
2.Downloading image file from cloud storage ...
3.Image file successfully downloaded from cloud storage ...
4.Importing image file into image catalog ...
Here are some possible points for investigation and troubleshooting:
1.Issues with the specifications or format of the image file
Ensure that the uploaded image is in a format supported by PowerVS.
2.Corruption in the image file
Check the upload history in the ICOS bucket to ensure there were no errors during the upload process.
Calculate the MD5/SHA hash value of the image file and verify that it matches the hash of the original file to confirm file integrity.
3.Incorrect bucket settings
Verify that the ICOS bucket's region matches the region of the PowerVS instance.
Check the IAM policy and ensure that the bucket has "read" and "write" permissions assigned.
4.Network or temporary server errors
Temporary internal errors or traffic issues on the network could be the cause.
Retry the image import at a different time to avoid network congestion or service overload.
5.Issues on the IBM Cloud side
There might be unforeseen errors or problems with the image import process on the backend of IBM Cloud.
As Ricardo said, I also agree that problems can sometimes occur on the cloud side, so if the error persists even after retrying, I recommend opening an IBM Cloud support case.

I've also run into issues at times when exporting and importing images on PowerVS - it may be related to something on the backend.
I recommend opening an IBM Cloud Support case; they'll be able to investigate and either guide you through the fix or resolve the backend problem directly.

Now, it seems like it's going to finally do it's thing...all green.

Then few minutes later, it puked and another error popped up with the error message more cryptic than previous error.

Any idea?

If you've configured access restrictions using the ICOS firewall,
access from PowerVS may be blocked by the ICOS firewall.
In this case, you'll need to allow the source IP address used by PowerVS.

It is my first attempt to create an instance and I've gotten as far as uploading the OS image. I created a bucket and uploaded my OS image and when I try to import the image, it comes back with the following error:
