This issue has occurred with multiple tape cartridges using the same drive. Now the drive is also generating a Drive alert 31 as well.
I can't find the document or the LED names of the said Drive (IBM LTO6 Drive, Part Number 8-00974-11). Attached are the pictures of a similar drive for your reference. In the front view, the status LEDs are (Blinking blue, Off, Blinking Green) which were normal status. In the backview however, the LED in the bottom center has become amber.
Can you please guide if this indicates harware failure. Or it is something recovereable, as this drive has not been cleaned by a cleaning cartridge regularly.

Does this issue occur when the drive attempts to verify multiple physical cartridges, or just the same (single) cartridge?
May be useful to eliminate an issue with the actual tape cartridge that the drive is attempting to mount. If this repeatedly occurs with the same tape, then could you try and perform the verification using a different drive?
From IBM drive perspective, alert 3 could indicate an unrecoverable read positioning error, which could possibly be down to a media error.
