    Anyone out there with experience transforming multiple response variables (e.g., yes, no, don't know, refused) in the Behavioral Risk Factor Surveillance (BRFSS) data into a binary yes/no and deleting the don't know/refused categories?

    yes, no, don't know, refused - are not multiple response, they are alternative variants of the single response: mutually exclusive.
