That does not sound right.
Please raise a support case and have our support team help you out.
PS. In general I would advice against using entitlement workflows as this may conflict if you want to automate provisioning - i.e. entitlement/account request workflows is only for manual provisioning and was part of the original product originally but now with the full operational workflows I would handle approvals etc. there (just as an example - remember to always test your approvals with rejections and afterwards do a dummy change to person...)
