Last week, I discussed the Post-Incident Review in the 30-sec SRE series. This week, I wanted to complement this article with an important topic that one needs to be aware of: Cognitive Bias, a systematic pattern of deviation from norm or rationality in judgment. Individuals create their own "subjective reality" from their perception of the input. As we construct the narrative behind an incident, we must base it on facts to lead us to accurate conclusions and effective solutions.
