The icon with the Cognos logo is missing on the front page after an upgrade form Cognos 11.1.7 to Cognos 184.108.40.206
The filename is common-CA_Avatar_Colour_64.
There are no other problems with images on this instance.
Be sure to empty the browser cache after upgrade. I´ve seen that a lot, after upgrading, and it needs to be the entire cache not just for last 24 hours or so.
If thats not working, make sure you also upgraded the IIS with the setup script.
