SPSS Statistics

Hello,

I am having trouble recreating the table below. Specifically I don't understand what regression to run for each column. I understand for each column the dependent variable is continuous and that each predictor listed below (i.e Age, race, etc) are either nominal or continuous as well. However, I don't understand what test was used to list all of the adjusted odds ratios for complications column and to list all of the percent change with confidence interval for the total hospital charges and LOS columns. I have already log transformed hospital charges and LOS and have all the variables listed in my dataset (although RACE and INSURANCE are multilevel nominal categories and I don't know if I need to create dummy variables for them). 

Any help with this is highly appreciated, as I've spent hours now trying to understand what direction I should take but keep finding myself lost.
