It seems the disk was mklvcopy'd not mirror'd. Anyway that was last time .. now we have a new failed disk and I just want to know what to do better this time!

how do I know if it was mirror'd or mklvcopy'd ? So I know what to do to recreated once the disk is replaced.

btw ... i was nowhere near when this was done ... that admin is long moved on

    This time hdisk3 is bad

The real issue here, however, is that you are listing vg01 so I'm not sure how we can look into this further for you?!?!?!

And I can't help but to ask why you put a paging space in vg01? They should normally be in rootvg.

I was stunned last time ... I want to be better prepared this time.

To make things clean, you can rmlvcopy (e.g. rmlvcopy lvol1 1 hdisk3) all the LVs out of hdisk3 (lspv -l hdisk3); rmdev -dl hdisk3; replace it with a new one, cfgmgr, extendvg vg01 hdiskX, and put the mirror back to hdiskX.

If you just swap out the hdisk3 w/o cleaning up the ODM, AIX is not happy. And depending on the exact type of disks you have, the procedure might need to be altered a bit to fit your local need. Good luck!
