I did several VIOS installations from HMC - had some problem as well but finally I somehow figured it out -- the command I run was:

    installios -d /tmp/Virtual_IO_Server_V2.2.1_DVD_1_of_2_102011.iso -i VIOS_IP -g VIOS_GW -S VIOS_MASK -P 1000 -D full -n -l en_US -A eth0 -s MANAGED_SERVER -p LPAR_NAME -m VIOS_MAC_ADDRESS -r VIOS_LPAR_PROFILE_NAME
