I have one process which has a few steps. While viewing it from MWS 8.2.1 Administration–> Business —> Business Process, one step image is missing from the process diagram. By looking into its instance execution step, each step/transition was executed. However, the process instance diagram is missing step image.

Did anybody have the similar experience?

Just a quick update on my question in case anybody ran into the same issue.

The Monitor UI MWS patch MON_8.2_SP1_Fix6 does not solve the issue regarding the incomplete process diagram. I finally decide to rollback all Monitor UI MWS fixes to none after a few days struggling. The issue is resolved. Everything looks good in both process detail page and its process instance.

I think MonitorUI fixes cause additional issues. Fortunately, 8.2 Update Manager allows me to uninstall the fixes pretty easily.
In short, do not install any new fixes unless they do solve your issue.
