2) remove/replace adapter (it should perform a rmdev -dl on the device, but it has to be down-detached to do this)
3) configure replaced adapter (it really just runs cfgmgr)

It sounds like it may not have removed it properly or that the ODM is confused.

I would try to down/detach the adapter. Remove it from the OS via rmdev -dl and then boot the box. It should re-add it when it performs cfgmgr on boot. If its still in this state, I would guess that the ODM may be the issue.
