I checked the level of each patch sets and some patches are applied, but others are not.

Firstly, it would appear that you have a logically bogus version of /usr/bin/rpm. Do you know how it got onto your system? Did it ever work in the past? I would try installing a working version and then try your upgrade over again.

To me it looks like a kernel symbols mismatch which is fairly common if you update the kernel lpp and don't reboot.
