Classification members not showing on form

    I have a classification call Facility Status with four values. This classification is on a form, but no values are displayed for it whenever the users tries to select. I've seen this problem a few times before without ever figuring out the cause. In those previous instances, just opening and saving the classification cleared up the problem, but not this time. I've also tried opening and saving each value under the classification, deleting and recreating, clearing cache with and without hierarchy rebuild, even rebooting the servers, but I haven't been able to get this to work again (it formerly worked just fine, but ceased working at some unknown point in the past).
